Dog Training Waiver and Intake Form

Please fill out this form if you are interested in any of my dog training services. Please fill out any relevant information, training goals, or issues about your dog. One form per dog is required if you enroll multiple dogs for any of my training programs.

A Form must be provided prior to services being rendered. There are no exceptions. A new form is required for each training program or when your rabies expires. If you do individual lessons, you are responsible with keeping your information up to date.

What are your training goals? Are there any particular issues that I should be aware of? Are they good with dogs, cats, people, kids, etc.? Are they on any medications or do they have any health concerns? Are you interested in reactive dog training?

By submitting this form, you declare that you are the legal owner of the animal(s) listed herein, or their authorized representative, and that the animal is free of any parasites, healthy enough for the services rendered, and has not been exposed to any infectious diseases within 30 days of your service date. You also agree that any boarding dog is up to date on all required vaccines (including leptospirosis) and preventative care, including flea and tick and heartworm prevention. If your dog is under-vaccinated you assume any risks associated with this choice and JK9 Pet Services, LLC will not liable for any health or other issues arising from lack of vaccinations or relating to your dog's medical history (known or unknown).

You agree that you have disclosed any and all behavioral, medical, or other relevant concerns relating to your pet. You also agree to pay the "reactive dog training" fee if your dog is reactive/aggressive towards other dogs.

You have acknowledged any and all risks associated with the services rendered by JK9 Pet Services, LLC and that if an emergency should occur that JK9 Services, LLC will be granted permission do whatever is deemed necessary for the health, safety, and well-being of your pet, including seeking professional medical treatment at the Owner's expense. JK9 Pet Services, LLC will attempt to contact the Owner and/or the provided Emergency Contract at least once prior to taking action.

You agree that any damages or injuries caused by your pet while under the care of JK9 Pet Services, LLC are the Owner's responsibility and JK9 Pet Services, LLC shall not be held responsible for any injuries or expenses incurred. You also agree to waive JK9 Pet Services, LLC from all liability or expenses, seen or unforeseen, that may occur as a result of services rendered. If your dog injuries themselves or another dog while under my care, you will be responsible for any and all veterinary bills and expenses related to the incident.

You agree to pay your invoice by the listed due date provided on your invoice. Any outstanding balance not paid within 5 days of the invoice due date are subject to a 30% fee in accordance with my cancelation policy. You also agree that you have read and will abide by all of the terms listed in my cancelation policy.

By enrolling in services provided by JK9 Pet Services, LLC, your dog(s) photos and videos will be used for marketing or advertising purposes and will also be posted on social media.

By checking this box, I agree to the terms stated herein. I also agree that my dog is healthy enough for boarding and/or training, fully vaccinated, on recommended prevention, and parasite/contagion free. I realize that by entering my name below, this will act as an electronic signature.
